By opting in to receive SMS messages and scheduling an appointment or receiving services at Smile America Dental Center, you agree to these terms and conditions.
1 – SMS Messaging Service
By providing my phone number, I consent to receive SMS text messages from Smile America Dental Center for appointment reminders,
marketing messages, and general two-way communication. Msg frequency varies.
Msg and data rates may apply. Reply HELP for support. Reply STOP to opt out.
2 – Message Frequency
You will receive up to 4 messages per appointment scheduled.
3 – Message and Data Rates
Message and data rates may apply based on your mobile carrier’s terms.
4 – Privacy Policy
Your information will be handled in accordance with our Privacy Policy.
5 – Opt-Out Instructions
You can opt out at any time by replying “STOP” to any SMS message. Reply HELP for support. You may also contact us directly at (248)478-0900
6 – Liability
We are not responsible for any charges, errors, or delays in SMS delivery caused by your carrier or third-party service providers.
By opting in, you confirm that you are the owner or authorized user of the phone number provided and that you are at least 18 years old.
Services Provided
Smile America Dental Center provides comprehensive dental care services, including but not limited to:
★ Preventive care (cleanings, examinations, fluoride treatments)
★ Restorative dentistry (fillings, crowns, bridges)
★ Periodontal treatment
Patient Responsibilities
Patients are responsible for:
★ Providing accurate and complete health information
★ Following pre and post-treatment instructions
★ Maintaining good oral hygiene
★ Keeping scheduled appointments or providing adequate notice for cancellations
★ Payment for services rendered
Appointment Policy
★ Appointments should be canceled at least 24 hours in advance
★ Repeated no-shows or late cancellations may result in a cancellation fee
★ Emergency appointments will be accommodated when possible
Limitation of Liability
★ Smile America Dental Center’s liability is limited to the direct cost of services provided. We are not liable for indirect, incidental, or consequential damages.
Modifications
These terms may be updated periodically. Patients will be notified of significant changes.
